If the player has any medical conditions it is important that you record these on the form and notify the team coaches. If they require any medication, such as asthma inhalers, please ensure these are bought to all training and matches and the team coaches are aware of their location.

The membership fees for this season will be £100.00. Payment information is included on the membership form, please rember to include the players full name and team they are signing for when paying via BACS.

Can we also ask that you print & hand in the completed forms to the team manager / head coach.

Please note a slight change to this years sign up conditions, where all our returning & all new sign ups who have already started training are required to pay no later then 30th September 2026.
